Mix all alcoholic ingredients in a shaker with ice. Shake well and then add milk or cream and shake lightly. Pour over ice in a martini glass.

This drink can also be garnished with either a cinnamon stick, a chocolate swizzle stick, or even two soft caramels skewered and placed on the rim of the glass.
